In order to be eligible to test, you must complete the following to its entirety:

• To test on 11/11/17

  • The main training course must be completed by 11/4/17. EVERYTHING must be checked off and completed without skipping any step or activity.
    • Manager students: Manager Training V2
    • Caregiver students: Either Caregiver Training V2, Caregiver Training A1, or Caregiver Training E1
    • CNA-CG Bridge students: CNA-Caregiver Bridge V2
  • Entire balance needs to be paid in full and all required documents must be turned in to ALTS and completed by 11/4/17
    • Manager students: drivers license
    • Caregiver students: drivers license, food handler's certificate or card, 16 hands-on training waiver, 16 hands-on training checklist
    • CNA-CG students: drivers license, food handler's certificate or card, Arizona CNA license
  • The On-Site Only Group Review and On-Site Only Practice Exams must be completed no later than 5pm on 11/10/17

If you do not meet the deadlines listed above, you will NOT be eligible for the State Exam, NO EXCUSES.

We must submit proof that you finished your entire course by the deadlines listed above to the State. We will NOT accept any late or last minute students for the State Exam. Thank you.
