Hello students,

ALTS office will only be open to those students who are scheduled to take the State Exam on 12/16/17. Other students who want to come in to work on their program can come in no earlier that 12 pm Noon.

• To test qualify for the State Exam on 12/16/17:

  • The main training course must be completed by 12/9/17. EVERYTHING must be checked off and completed without skipping any step or activity.
    • Manager students: Manager Training V2
    • Caregiver students: Either Caregiver Training V2, Caregiver Training A1, or Caregiver Training E1
    • CNA-CG Bridge students: CNA-Caregiver Bridge V2
  • All required documents must be turned in to ALTS and completed by 12/9/17.
    • Manager students: drivers license
    • Caregiver students: drivers license, food handler's certificate or card, 16 hands-on training waiver, 16 hands-on training checklist
    • CNA-CG students: drivers license, food handler's certificate or card, Arizona CNA license
  • The On-Site Only Group Review and On-Site Only Practice Exams must be completed no later than 5pm on 12/15/17.

If you do not meet the requirements listed above, you will NOT be able to take the State Exam on 12/16/17, NO EXCUSES.
