State Exam: Check-in 12:30pm, Test begins 1pm on 1/21/18

In order to be eligible to test, you must complete the following in its entirety:

• • The main training course must be completed by 01/15/18. EVERYTHING must be checked off and completed without skipping any step or activity.
    • Manager students: ALTS Manager Training
    • Caregiver students: Either Caregiver Training V2 or Caregiver Training A1
    • CNA-CG Bridge students: ALTS CNA-Caregiver Bridge
  • All required documents must be turned in to ALTS and completed by 01/15/18
    • Manager students: drivers license
    • Caregiver students: drivers license, food handler's certificate or card, 16 hands-on training waiver, 16 hands-on training checklist
    • CNA-CG students: drivers license, food handler's certificate or card, Arizona CNA license
  • The On-Site Only Group Review and On-Site Only Practice Exams must be completed no later than 5pm on 01/19/18
