Hello students,

The next State Exam will be on 02/08/20. Exam starts 8am, check-in will be 7:30am. Please bring your student badge and photo ID at check-in. Please see below on how to qualify:

• Tuition paid in full no later than 02/03/20 by 3pm
• All required documents filled out and submitted in your Orientation and Document Submission course no later than 02/03/20 by 11:59pm. 
  • Caregivers: 12 hours hands-on training form, Food Handler's Card, Photo ID, and Social Security Card.
  • CNA-Caregiver Bridge: AZ CNA License, Food Handler's Card, Photo ID, and Social Security Card.
  • DCW-Caregiver Bridge: Article 9, Food Handler's Card, Photo ID, and Social Security Card.
  • Managers: ID and Social Security Card.
• All activities (exams, quizzes, videos, and chapter readings) completed in full no later than 02/03/20 by 11:59pm. The ONLY activates that are due no later than 02/07/20 by 5pm are your Practice Exams, which are located at the very bottom of your training program.
• Fill out the State Exam Application Form in your Orientation and Document Submission course, due no later than 02/03/20 by 11:59pm. We will not accept any verbal or email confirmations for the State Exam. You MUST fill out the State Exam application form for ALTS to review your activity and to confirm your scheduling.
• You must have ALL required classroom hours logged into our time clock at the ALTS office. If you are short hours you will not be scheduled to test. Please see a staff member no later than 1/31/20. CGs 30 hours, CNA-CGs 12 hours, DCW-CGs 18 hours, MGRs 40 hours required in the classroom.

We will NOT accept any last minute submissions. If you do not meet the deadline and the qualifications, you will not be scheduled to test. If you go past your maximum time of enrollment, there will be a $150 extension fee added per each month needed of extension.
