The State Exam is on Saturday, April 18, 2020.

State Exam Times:

If your email from Headmaster has you scheduled at:

8:00 am, pleas check in at 7:30 am

9:45 am, please check in at 9:15 am

Please see below on how to qualify:

• Tuition paid in full no later than 04/13/20 by 3pm
• All required documents filled out and submitted in your Orientation and Document Submission course no later than 04/13/20 by 11:59pm. 
  • Caregivers: 12 hours hands-on training form, Classroom TIme-Sheet (30 hours), Food Handler's Card, State Exam Application Form, Photo ID, and Social Security Card. 
  • CNA-Caregiver Bridge: AZ CNA License, Food Handler's Card, Classroom TIme-Sheet (16 hours), State Exam Application Form, Photo ID, and Social Security Card.
  • DCW-Caregiver Bridge: Article 9, Food Handler's Card, Classroom Time-Sheet (18 hours), State Exam Application Form, Photo ID, and Social Security Card.
  • Managers: ID, Social Security Card, Classroom Time-Sheet (40 hours), State Exam Application Form.
• All activities (exams, quizzes, videos, and chapter readings) completed in full no later than 04/13/20 by 11:59pm. The ONLY activates that are due no later than 04/17/20 by 4:00pm are your Categorized State Exam Practice Exams course, which are located at the very bottom in your Dashboard.
• Fill out the State Exam Application Form in your Orientation and Document Submission course, due no later than 04/13/20 by 11:59pm. We will not accept any verbal or email confirmations for the State Exam. You MUST fill out the State Exam application form for ALTS to review your activity and to confirm your scheduling.
• You must have ALL required classroom hours logged into our time clock at the ALTS office. If you are short hours you will not be scheduled to test. CGs 30 hours, CNA-CGs 12 hours, DCW-CGs 18 hours, MGRs 40 hours required in the classroom. Due to the COVID 19 virus, all acquired hours obtained before our closure will be added to the Classroom TIme Sheet that you will need to fill out and upload to your Orientation and Document Submission course no later than 4/13/20 by 11:59pm.

We will NOT accept any last minute submissions. If you do not meet the deadline and the qualifications, you will not be scheduled to test. If you go past your maximum time of enrollment, there will be a $150 extension fee added per each month needed of extension.
